This page is only for reporting scores to One Adoption West Yorkshire.
As part of the terms and conditions of the ASGSF outcome measure tools (OMT) scores have to be provided. OAWY are responsible for collecting these scores from therapeutic providers and submitting them to the ASGSF portal. Each application needs to have the scores for at least one of the below outcome measure tools.
You will need the ASGSF reference for the relevant application. You can find this on your contract.
What are they asking for?
The ASGSF ask for a baseline and an endpoint score for each application.
What if therapy is ongoing?
Ongoing therapy can span multiple blocks of ASGSF funding and have multiple applications associated with it. You will receive a new contract and new ASGSF reference for each funding application for a family/child.
A midpoint score can form the endpoint of one application and the baseline of the next if the therapy is ongoing. We ask you to submit each separately (one baseline, one endpoint), alongside the relevant ASGSF application reference.

- Total Clinical Score (Maximum 204)
- Total Low Self-Esteem Scale Composite Score (Maximum 36)

The ASGSF require a minimum of 3 goals but we are able to report up to 5.

We know that it’s not always possible or appropriate to complete an OMT with a family. They may be in crisis, the therapy might have ended unexpectedly, or the family may refuse to complete one. We still need to know if this is the case.
